THRIVE, a new medically supervised program offered by Appalachian Regional Healthcare System, transitions participants to wellness. The program helps individuals manage their acute chronic disease through supervised exercise, nutritional counseling, chronic disease education, self-management strategies and action planning to meet their individual wellness goals.
Participants are evaluated and assigned to either the Rehabilitation Group or the Wellness Group. The Rehabilitation Group focuses on individuals with musculoskeletal issues, pain management needs, and balance problems. The Wellness Group focuses on individuals with Chronic Diseases such as Cardiac or Pulmonary diagnoses, and Diabetes.
The THRIVE multidisciplinary team collaborates with the participant's physician and measures outcomes to ensure participants successfully transition to living a healthier lifestyle. Program sessions are held three times each week, for 12 weeks at the Wellness Center in Boone. The cost of the program is $75 per month. The first group is scheduled to begin June 13, 2011
Individuals may refer themselves, but referrals are also accepted from all health care providers. Individuals who self-refer will be required to get a signed medical release form from their doctor prior to beginning the program.
